Флегмона червеобразного отростка представляет собой сложную форму острого аппендицита, характеризующуюся формированием воспалительного инфильтрата, возникающего, когда защитные механизмы организма успешно ограничивают локализованную перфорацию аппендикса. Этот процесс обычно развивается вследствие обструкции просвета, ишемии, некроза и последующего разрыва стенки отростка. Дифференциальная диагностика между флегмоной и абсцессом представляет собой серьезную клиническую задачу, поскольку именно от этого зависит тактика лечения. Компьютерная томография (КТ) остается методом выбора, позволяя визуализировать нечетко очерченные воспалительные массы без выраженных полостей с жидкостью. Тактика лечения остаётся предметом дискуссий и колеблется между консервативным подходом (антибактериальная терапия, наблюдение, возможная интервал-аппендэктомия) и ранним хирургическим вмешательством. Современные данные подтверждают эффективность консервативного лечения, которое ассоциируется с меньшим числом осложнений и снижением необходимости экстренной операции. Тем не менее, развитие лапароскопической хирургии вновь актуализировало интерес к ранней аппендэктомии в отдельных случаях. Необходимость выполнения интервал-аппендэктомии пересматривается: предпочтение отдаётся избирательному подходу. Визуально-контролируемое дренирование играет ключевую роль при лечении абсцессов, хотя и сопровождается определённым риском неудачи. Оптимальный выбор антибиотиков, продолжительность терапии и новые данные о роли микробиома аппендикса продолжают формировать современное понимание заболевания. Сохраняющиеся пробелы в диагностических критериях и стандартах лечения подчёркивают необходимость проведения масштабных многоцентровых рандомизированных исследований для выработки доказательных клинических рекомендаций по ведению пациентов с аппендикулярной флегмоной.
